The member record

Open work first, in the words the team used.

The record leads with what isn’t finished, earliest first, and who it is waiting on. Notes read like people talk: “He agreed to meet at the encampment Friday morning.”

Assign or change the case manager, log an outreach attempt, and see the care plan without leaving the page.

Care plans

Built to pass review, and honest about the gaps.

  • Each section says what a reviewer looks for before the worker fills it in.
  • Member strengths, preferences and cultural and linguistic needs are recorded in the member’s own words.
  • Consent standing is read from the consent register, so it is never a status somebody has to keep up to date.
  • Missing signatures, and needs with no goal written against them, are listed as loose threads, not hidden.

Housing

Every housing case, where it stands, what moves next.

  • Housing Navigation, Housing Deposits, Transitional Rent, and Housing Tenancy and Sustaining Services.
  • The individualized housing support plan, deposit itemization against the $7,500 cap, the Transitional Rent stay count and the tenancy assessment are built in.
  • The deposit cap is shown as a running total, without blocking the work.
  • The product says plainly what it doesn’t verify, and that sending the packet and the plan’s decision happen outside it.

Screening & Assessment

One screening, every way a member qualifies.

Choose what you are screening for: Enhanced Care Management, and the housing and rent Community Supports such as Housing Navigation and Housing Deposits. The questions adapt. Workers can have any question read aloud, or hear it in Spanish.

Nothing leaves incomplete

A packet with gaps lists them. Medi-Cal CIN, date of birth, housing situation: whatever the plan will need.
